Story of Balaan and his encounter with the Angel who confronted him on the road that day in his travels to Moab. Numbers 22-23
Balaan knew of the Hebrews God which was not unusal in that day nor is it today...for unbelievers to know of our God but not to know Him.
At any rate...the Angel confronted Balaam who promptly...obviously out of fear...admitted he had sinned to keep the angel from killing him.
My question is this...how can we tell when someone's profession of faith is real or just out of fear or need?

Not at all, and the answer is: By their fruit!

How one lives their life, how one conducts their business, how one interacts with friends and family and etcetera is a reflection of the Spirit that's in them. Remember though, we're still human beings in a mortal body and we still sin. We don't do the right thing all the time and that has to be taken into consideration.
There are unsaved people who are generally good folks, but talking to them will usually reveal their thoughts on the Lord.

but by the same token...I have met people who have done bad things who when I started really talking to them...were saved.
They had just royally screwed up.

He has prepared for him the instruments of death (Psa 7:10-13); That's sort of a heavy scripture. God has already for the wicked the way by which he is going to die. "He's bent his bow, he has sharpened his sword, he already has planned the method of the destruction of the wicked."
